Mat Mania is a pro-wrestling arcade game where players take control of a wrestler as they fight through a series of increasingly difficult opponents to eventually claim the championship title. The game is styled as a televised wrestling broadcast, with comedic intermissions featuring the chaotic wrestling host, Cory (or Nari in the Japanese version), who introduces each match in a manic and humorous way. The player starts by challenging various colorful and unique opponents, each with their own fighting styles and personalities. After defeating all the contenders, the player faces off against the reigning champion in the fifth and final match of the championship journey. Upon winning the title, a special ceremony awards the player a championship belt. Once crowned champion, the player must defend their title in a series of rematches against the same five opponents, who are chosen randomly. The goal is to maintain the championship by defeating all challengers and holding onto the title for as long as possible.
