The player controls an unnamed hero on a mission to rescue Princess Mariko from the fortress of the evil warlord Akuma. The hero must use punches, kicks, and dodges to defeat enemies, with both the player and enemy having health bars that deplete with each successful hit. Health regenerates slowly if the fighter avoids taking damage for a short period.
In addition to human enemies, the player faces environmental hazards like cliffs and falling portcullises, and occasionally, Akuma''s hawk attacks the hero, which can be deflected with precise timing. The game includes cutscenes depicting Akuma''s orders and Princess Mariko''s anxious wait.
As the hero progresses, he eventually confronts Akuma in a final battle and then enters Mariko''s cell to rescue her. However, if the hero approaches Mariko while in a fighting stance, she kicks him, resulting in instant death. Only by approaching her calmly, without fighting, can the hero successfully rescue her, and the game ends with the two of them leaving the fortress together.
The game blends martial arts combat with platforming challenges, requiring both skill and strategy to successfully navigate the fortress and complete the rescue.
